on the Latest National
Report on Diet and Exercise
On September
5, the Institute of Medicine
released a 1000-page report
offering Americans its
recommendations for maintaining
normal weight and general
health. IOM is a private
not-for-profit organization
that provides health policy
advice to the National
Academy of Sciences. The
study on which the report
is based was undertaken
by the IOM's Food and
Nutrition Board. It is
a revision of dietary
guidelines published more
than a decade ago. It
was prominently reported
in the news media, with
many headlines noting
the report's call for
an hour of exercise a
day -- twice that called
for in previous guidelines.
Here are some highlights:

Adults
should get 45 -- 65%
of their calories
from carbohydrates,
20 --35% from fats,
and 10-35% from protein.
These ranges are a
change from earlier
guidelines that set
less flexible levels
at 50, 30, 20 respectively. |

Diets too low in fats
and high in carbohydrates
deplete HDL, the "good
cholesterol. Diets
too high in fats lead
to obesity. |
 |
The report gives guidelines
for diet and exercise
according to specific
genders, ages, weights
and levels of physical
activity. E.g., a
30-year-old woman,
5'5" who lives
a sedentary lifestyle
should consume between
1800 and 2000 calories
a day. More for the
more active person. |

The new one-hour-a-day-total
exercise goal stems
from studies of how
much energy is expended
on average each day
by individuals who
maintain a healthy
weight. Energy expenditure
is cumulative, including
both low-intensity
activities of daily
life, such as stair
climbing and housecleaning,
and more vigorous
exercise like swimming
and cycling. Someone
in a largely sedentary
occupation can achieve
the new exercise goal
by engaging in a moderate-intensity
activity, such as
walking at 4 miles
per hour, for a total
of 60 minutes every
day, or engaging in
a high-intensity activity,
such as jogging for
20 to 30 minutes four
to seven days per
week. |

Sugars should comprise
no more than 25% of
total calories consumed.
But the report distinguishes
natural sugars found
in foods (fruits)
from sugar introduced
into candies, soft
drinks, pastries,
and other sweets.
The latter are of
no nutritional value. |

The report also distinguishes
different fats: Saturated
fats found in meats,
butter, whole milk
are to be limited
because they boost
cholesterol in the
blood. Monounsaturated
fats and Polyunsaturated
fats are found in
vegetable oils, fish,
and ought to be substituted
for other fats. Trans-fats
are those found in
fast foods and snacks
and have no nutritional
benefit. |

Dietary fiber intake
levels are recommended
for the first time
because studies have
shown that diets low
in fiber add to the
risk of heart disease. |
